Your Summer Collie Puppy from Desert Kennels-
​A few details specific for this year's litters, but good general information for all of my puppies and dogs that go home in the summer.

​A blank Collie Litter Eye Exam form- my local vet's favorite.

MDR1 Status Form-
For all breeds with the MDR1 risk.  A form with the problem drug list and your dog's MDR1 gene test status, for your vet's files and your own travel folder.  
​MDR1 Short Form but an older list includes some recommendations for veterinarians.
​MDR1 Dosage Details from Washington State University (this is a link to WSU Problem Drugs page on their site).
​Personally I travel with all 3 MDR1 forms and I have politely asked my regular veterinarian to keep a copy of all 3 lists in my file.

Registration Types-
​American Kennel Club and United Kennel Club registration types and benefits listed.  Limited for pets vs Full for breeding dogs, etc.

​2020 Puppy Diets-
​This still says "Doberman" thru out it but the recipe is the same for my Collies and Pyrs. 
​The Collies eat a bit less than Dobermans at the same age and the Pyrs will eat a bit more.

​Record of Shots and Worming-
​Enough spaces for the vaccine stickers if you do your own shots for the life of your dog.  Includes the vaccine and worming schedule we use here for our dogs.
